To get a copy of a report, fill out the department's online Records Request form, or print the form and bring or mail it to Cotati Police Department, Attn: Records, 203 W Sierra Avenue, Cotati, CA 94931. You will need photo ID to pick it up. Once a request is approved and the report is available, it takes about 7 to 10 working days. Drivers, passengers and registered owners in a collision are entitled to the report, and your insurance company may request it for you.

How do I make an injury claim against the City of Cotati?
The Office of City Manager and City Clerk handles claims management for the City. The City states that a claim must be presented as the California Government Code requires, by the person making it or someone acting for them. City Hall is at 201 West Sierra Avenue, open Monday to Thursday, 7:30 AM to 5:30 PM (closed 12:00 to 1:00), 707-792-4600.
Timing is strict. A claim for injury against a city must be presented within six months (Government Code 911.2), and the City then has 45 days to act on it (Government Code 912.4). Where would a Cotati injury case be filed?
For an injury or a death, the superior court in the county where the injury happened, or where a defendant lives, is a proper court (Code of Civil Procedure 395). Which agency responded. Cotati police handle most incidents in the city, but the CHP may handle a crash on a road it patrols, and the Office of Traffic Safety notes that a crash the CHP reports on a shared road does not count against the city. Ask the officer which agency took the report.

What could this look like? An example
For example, imagine a commuter walking from the SMART Cotati station on East Cotati Avenue who is struck in a crosswalk by a turning car. A Cotati officer takes the report and gives her a report number. Ten days later she submits the department's Records Request form online and picks up the report with photo ID about a week and a half after it is approved.
The crosswalk's faded paint makes her wonder whether the City shares responsibility, so she presents a claim through the Office of City Manager and City Clerk at City Hall on West Sierra Avenue within six months, while her claim against the driver's insurer goes forward.
